All CQC program activities will be documented by keeping hard copies of each of the reports and forms in a clearly labeled individual binder or file folder. All QC documentation will be completed using the Daily QC Report.
Daily QCDocumentation Forms
The forms listed below will be completed on a daily basis by the CQC program manager or a CQC staff member. Hard copies of each form will be maintained in the field in three-ring binders or file folders with clearly labeled sections for each form. The CQC program manager will also complete KBR’s Quality Assurance Representative’s daily and weekly reports.
Daily QC Log-in – The daily log-in will be prepared by the CQC program manager to provide a record of the inspection date, the name of the inspector, the amount of precipitation in the last 24 hours, weather conditions and temperature.
QC Narrative – The QC narrative will be completed on a daily basis and used to record the inspection date and detailed notes on daily activities including such things as daily field inspections, receiving inspections, work delays, instructions received, and general comments.
Activity Related QC Documentation Forms
The forms listed below will be completed as required by the activities and occurrences of each particular day. The forms will be completed by the CQC program manager or a CQC staff member.
Activity Start/Finish Log – This log will be used to record the actual start and actual finish dates of each work activity.
Preparatory and Initial Inspection Log – This log will be used to record the planned date and actual date of each preparatory and initial inspection.
QC comments are used to record, track and close out deficiencies. This log will be used to record a number for each deficiency, the associated work activity, the inspector, notes on the matter, and the status of the deficiency (i.e., pending, corrective action included …, or matter resolved ), the date of the report, and the date the deficiency was identified. Completing this form ensures correction of deficiencies by requiring documentation of how the problem was resolved.
Quality Control Testing
This log will be used to record all QC Testing. Information to be recorded will include the QC test number, the specification number and paragraph, the activity number, the lag/lead time, the name of the person conducting the test, a description of the test, and QA verification.
Installed Property Information
This log will be used to record information on equipment installed in association with a work activity. Recorded information will include a unique number for each piece of equipment, the specification section and paragraph, the activity number, the lag/lead time required for installation, the installation number, the unit cost, the manufacturer identification number, the equipment serial number, the model, and a description of the piece of equipment.
Preparatory Inspection Report Form
The preparatory inspection form will be used to record findings during the preparatory inspection and allow for documentation of applicable specifications and contract drawings, equipment checks, physical examination results of required materials and equipment, hazard analysis and safety requirements, documentation of construction tolerances and workmanship standards.
Initial Inspection Checklist
This checklist will be completed when conducting and initial inspection. Recorded information will include the activity number, the inspector name, the inspection date, the work activity start date. A checklist of questions pertaining to materials being used, procedures being followed, work methods, the quality of workmanship, and control testing compliance will also be included.
INSPECTIONS, TESTING AND MONITORING
AI Andalus Company will conduct inspections, testing and monitoring throughout the project to ensure that operations are in compliance with contract requirements and that the final product will meet contract specifications. A list of each work activity and the tests and monitoring associated with each activity will be available on site.
INSPECTIONS TESTING AND MONITORING
Inspections, testing and monitoring activities to be conducted throughout the project include field inspections, field monitoring, field tests, laboratory tests, factory tests, receiving inspections, surveys, review of manufacturer’s certificates, compilation of checklists and equipment calibrations. A general discussion of each activity is presented below.
Field inspections may involve visual examination and possible measurements of materials and equipment being used, observations of techniques employed, or the inspection of a work product. Inspections are performed for the purpose of confirming that activities are conducted
in accordance with approved plans and contract specialisations.. Field inspections are performed on a daily basis for each work activity. The results of visual inspections are documented in the CQC daily report or on the appropriated inspection form or checklist.
Field tests are tests conducted in the field in connection with a particular feature of the construction and/or remedial action. Field tests are performed primarily on samples or portions of the as-built structure. Field testing is generally performed on an on-going basis to provide immediate confirmation that required characteristics are being achieved. Field tests are performed as soon as possible after material receipt or completion of the portion of the constructed work. This will provide prompt confirmation or rejection of the material or constructed work and minimise the potential of having to remove satisfactory work which has been added to defective material or work.
Laboratory testing is performed by off-site laboratories on samples of soil, materials, waste, or treated material. Laboratory testing is performed to characterise material or confirm treatment performance. Laboratory tests may also be used to confirm field monitoring and field test results. Laboratory tests are performed as soon as possible after samples are obtained to provide prompt confirmation or rejection of material and remediation or construction work.
